Forms of Identification

An individual must be a citizen of the United States in order to be eligible for Medicaid benefits. Approved documents for proving citizenship include a US Passport, Birth Certificate, Naturalization Papers, Alien Registration Card or a Final Adoption Decree.

Financial Resources

You also will need to show how much money you’re bringing in and what you own at the time of your application. Income and expenses are an important factor in determining eligibility for Medicaid. Bank account statements, rent payments, child support and pay stubs are all documents which will be factored in determining Medicaid eligibility.

Medicaid provides 100 percent coverage for most medical expenses and does not require payment of premiums or deductibles. In addition, health care providers who accept Medicaid cannot bill the patient for any additional charges after Medicaid has adjudicated the claim.
